Choosing A Form Template

The Form Builder is an Admin-only feature
The form builder feature contains pre-made templates from a few different categories: Pre-Participation, Wellness, Insurance, etc. Here's how to select from our templates.

00:01: Hi, this is Lindsey from Healthy Roster, and today I'm going to show you how to choose a template when using the Healthy Roster Form Builder. 00:10: To start click the document management tab 00:14: The next click forms 00:17: From here, you'll choose the add button to create a new form. 00:23: So this will bring you to the form template Gallery. You can either search for an existing form template or you can scroll down and click on the template you would like to use. 00:33: If you have questions or need more help, please visit the help tab on the left navigation or reach us directly at help@healthyroster.com.

Step-By-Step Tutorial:
  1. Login to the web version of Healthy Roster, and select the Document Management tab on your left navigation bar.
  1. Select Forms from the Manage Documents navigation menu.
  1. Select "Add."
  1. This takes you to the Form Template Gallery. Search for a template or simply click on a template to select the template you'd like to use.
  2. This takes you to the pre-made form with the option to tweak/customize it to your liking. Or, use one as-is and get this form attached to a document type to started collecting documents right away!
