Sending the Informed Consent for Telehealth Services

Please Note: Terminology is different based on your setting for example a clinical setting uses Care Group as Organizations, Patient instead of Athlete, and Visits instead of Treatment but the steps are still the same.

Step-By-Step Tutorial
  1. Sign in to your healthy roster account and click on the ‘Patients’ / 'Athletes' tab on your left navigation bar.

  2. Click on the ‘All Patients’ / ' All Athletes' tab

  3. Locate the patient you need to obtain consent from and click on the name. Here you will go to the ‘Documents’ tab and see the document outlined in red that needs to be completed.

  4. Hover over the form icon to the right of this document and select ‘complete form online’

  5. Fill in the Document Details and then click ‘Review and Sign’

  6. You will be prompted to enter your name and your role - select your signature type if you’d like, then click ‘I Agree’

  7. Review form and click the ‘sign here’ button

  8. You will be prompted to request a signature from the patient. Fill in the patient’s email and name, then click ‘Request Signature’

  9. Update Permissions as needed, then click ‘Update Permissions’
