Configuring A Survey

Sending surveys to patients allows for a more streamlined and effective way to gather important patient feedback or documentation and improves the quality of care. This tutorial will demonstrate how to configure your survey.

00:00: Hi, this is Nikki from healthy roster. 00:02: This tutorial is going to walk you through the action of configuring a new survey 00:07: surveys are able to be sent in healthy roster via email 00:11: or SMS. These surveys that are sent are based off of forms 00:15: that you've built within your form Builder. Let's review how to configure a survey. 00:20: You will Begin by selecting the documents tab on the left navigation. 00:25: Now select surveys 00:28: Because we are adding a new survey. We're going to select add. 00:33: Now you will provide your survey with a name. 00:37: The next action is to select which form your survey will deliver. 00:43: Now you will select okay? 00:46: Now review the survey definition you will see the name and the Forum assignment 00:50: that you've completed in the prior steps. 00:53: You will want to select the delivery type whether you're going to send via email 00:57: only or email and SMS. 01:02: Next you'll want to type in the request message. 01:05: This is the message that the patient will see once the survey is delivered 01:09: to them. This area is limited to 140 total 01:13: characters. 01:15: Next you will want to set the Privacy level for the documents that 01:19: are being completed via the survey entire care Circle 01:23: will be the default this insinuates. 01:25: Anyone that is able to view. This patient's documents tab will be able 01:29: to view the completed survey. Providers in primary is of course assigned 01:34: providers and typically the patient or the legal guardian as the primary user providers 01:39: only insinuates. Only provider users are able to view the completed document from 01:43: this survey. 01:46: Assigned to all insinuates that all of our organizations will be assigned 01:50: this survey uncheck the box and add each independent organization 01:55: as you wish. 01:57: The next two steps are for action plans and schedules. 02:01: This is not necessary to set up on the initial configuration. 02:05: We encourage you to review additional training materials to set up 02:09: action plans and schedules. 02:12: Now that the general Foundation of your survey has been configured. 02:15: You can select save as soon as you select save. 02:19: This survey is now live and can be delivered to patients within your account. 02:25: If you have questions or need more help, please visit the help tab on the left 02:29: navigation or reach us directly at help at healthy roster.com.

Step-By-Step Process:
  1. Select the Documents tab on the left-hand side of your screen
  2. Select Surveys
  3. Click Add
  4. Name your survey
  5. Next, from the drop-down, select the form that you created and wish to assign as your survey
  6. Click OK
  7. Next, set the survey definition. This will show the name and form you've selected prior, but also ask you to se the delivery type.
  8. Set the initial survey request message. This is the message that the patient will see immediately once assigned the survey. This text is limited to 140 total characters.
  9. Next select the privacy level of the survey
    1. Entire Care Circle: viewable to all assigned to patient
    2. Providers And Primary Users Only: Assigned providers and the Patient/Guardian
    3. Providers Only: Assigned providers only
  10. Assign organizations. The selection will default to all organizations; uncheck the box to assign to specific organizations.
  11. If you wish to create your action plan or schedules now, please move to Create a Survey Action Plan or Scheduling a Survey; otherwise, proceed to step 12. You can always edit each separately and do not have to have either set to send a survey.
  12. Select Save
