Adding Another Patient
If you are a parent/guardian of multiple patients and are not connected to all of them in Healthy Roster, please contact their athletic trainer via email with the following information:
- Email Address Already Linked to Healthy Roster (this is the email address that your existing account is connected to)
- Parent/Guardian First and Last Name
- Patient's First and Last Name
- Patient's Date of Birth
- Patient Gender
- Patient Graduation Year
The athletic trainer will connect you to your other children's patient Profiles. You only need one account to access all of your patients.
