Adding Another Patient

If you are a parent/guardian of multiple patients and are not connected to all of them in Healthy Roster, please contact their athletic trainer via email with the following information:

  • Email Address Already Linked to Healthy Roster (this is the email address that your existing account is connected to)
  • Parent/Guardian First and Last Name
  • Patient's First and Last Name
  • Patient's Date of Birth
  • Patient Gender
  • Patient Graduation Year

The athletic trainer will connect you to your other children's patient Profiles. You only need one account to access all of your patients.
