Reports for All Healthcare Providers

These reports are available to all Healthy Roster provider users.

Please Note: Terminology is different based on your setting, for example, a college setting uses Teams as Organizations and Units as Teams.
Please Note: You may notice a filter question on certain reports that states "include entries (injuries/treatments/referrals) that occurred before a patient was part of the Organization being reported on, but within the set date range, yes or no." If you have any backdated injuries for patients, you will want to click Yes to include that data.

Example: You added athlete to the organization on the date of 1/3/23, but the injury occurred on 1/2/23 - the default report will not pull this data unless you select Yes to the filter question above. The reports that include this are the following: injury log, detailed injury log, detailed treatment log, injury or treatment logs by org, injury trends, and follow up service report.

ENGAGEMENT REPORTS

INVITATIONS REPORT
  • PURPOSE: For auditing who has accepted their Healthy Roster invitation, including acceptance date; can also be used to see if a user has ““opted out” of creating an account
  • DATA PARAMETERS EXPLANATION: An entry is returned if the invitation created on date falls within the date range
CALL LOG REPORT
  • PURPOSE: For tracking calls made to physical Healthy Roster Kiosks; infrequently used by most providers
  • DATA PARAMETERS EXPLANATION: An entry is returned if the call was requested on a date that falls within the date range
CALLBACK REQUEST LOG
  • PURPOSE: For tracking callback requests related to physical Healthy Roster Kiosks; infrequently used by most providers
  • DATA PARAMETERS EXPLANATION: An entry is returned if the callback request was requested on a date that falls within the date range
INJURY STATION REPORT
  • PURPOSE: For providers who use injury station kiosks, this report will give you a list of the patients a provider has taken a call from within the specified date range.

TREATMENT REPORTS

SIMPLE TREATMENT LOG
  • PURPOSE: For tracking treatments administered (and to whom they were administered) in a certain time frame
  • DATA PARAMETERS EXPLANATION: An entry is returned if the treatment time falls within the date range
DETAILED TREATMENT LOG
  • PURPOSE: For tracking treatments administered (and to whom they were administered) in a certain time frame; includes body part; useful for analyzing Non-Injury Related Treatments
  • DATA PARAMETERS EXPLANATION: An entry is returned if the treatment time falls within the date range
TREATMENT COSTS
  • PURPOSE: For tracking the cost of billable and non-billable treatments by Organization
  • DATA PARAMETERS EXPLANATION: A treatment is included if the treatment time falls within the date range
TREATMENT COST BREAKDOWN (BY ORGANIZATION)
  • PURPOSE: For auditing the cost and amount of a particular treatment type broken down by organization
  • DATA PARAMETERS EXPLANATION: A treatment cost is included if the treatment time falls within the date range
TREATMENT COST BREAKDOWN (BY TREATMENT TYPE)
  • PURPOSE: For showing the clinical value of treatments administered; admins can edit the treatment value in the Admin section of Healthy Roster
  • DATA PARAMETERS EXPLANATION: A treatment cost is included if the treatment time falls within the date range
TREATMENT COST BREAKDOWN (BY INJURY)
  • PURPOSE: For showing the total cost of treatment for an athlete’s injury within the specified date range
  • DATA PARAMETERS EXPLANATION: A treatment cost is included if the treatment time falls within the date range

FOLLOW-UP SERVICES REPORTS

SIMPLE FOLLOW UP SERVICE LOG (BY PROVIDER)
  • PURPOSE: For tracking follow up services/referrals logged by a particular healthcare provider user in a certain time frame; does not include follow up service type
  • DATA PARAMETERS EXPLANATION: An entry is included if the service date OR the referral date falls within the date range
SIMPLE FOLLOW UP SERVICE LOG (BY ORGANIZATION)
  • PURPOSE: For tracking follow up services/referrals logged at a particular organization during a certain time frame; includes follow up service type
  • DATA PARAMETERS EXPLANATION: An entry is included if the referral was created on a date that falls within the date range
SIMPLE FOLLOW UP SERVICE LOG WITH SERVICE TYPE LOG (BY ORGANIZATION)
  • PURPOSE: For tracking follow up services/referrals logged at a particular organization during a certain time frame; does not include follow up service type; includes the background questions that were entered for those follow up services.
  • DATA PARAMETERS EXPLANATION: An entry is included if the referral was created on a date that falls within the date range
SIMPLE FOLLOW UP SERVICE LOG (BY ORGANIZATION WITH BACKGROUND QUESTIONS)
  • PURPOSE: For tracking important details about a follow up service/referral, including status, by Organization
  • DATA PARAMETERS EXPLANATION: An entry is included if the referral was created on a date that falls within the date range
DETAILED FOLLOW UP SERVICE LOG
  • PURPOSE: For a visually easier way to view the high level of detail for each Follow Up Service; includes all services listed in a single report; can take longer to load
  • DATA PARAMETERS EXPLANATION: An entry is included if the referral was created on a date that falls within the date range
FOLLOW UP SERVICE LOG (RAW DATA EXPORT)
  • PURPOSE: For transferring data to an external system to run analytics on the data for research; contains more information/codes than will be needed for most follow up service analysis
  • DATA PARAMETERS EXPLANATION: An entry is included if the service date OR the referral date falls within the date range

INJURY REPORTS

INJURY LOG
  • PURPOSE: For capturing all relevant Injury information and answers to Background Questions from Incident Reports; includes calculation of Days Out from Injury once injury status is marked “returned to play and closed”
  • DATA PARAMETERS EXPLANATION: An injury is included if the injury date falls within the date range
INJURY LOG BY TEAM
  • PURPOSE: For capturing all relevant Injury information and answers to Background Questions from Incident Reports; divided by team
  • DATA PARAMETERS EXPLANATION: An injury is included if the injury date falls within the date range
INJURY BREAKDOWN
  • PURPOSE: For identifying and following injury trends based off Background Question responses; populates pie charts related to the most common injuries with answers to Background Questions associated with them; ideal for looking at injury trends across a healthcare system/provider
  • DATA PARAMETERS EXPLANATION:
    • Injury and Graduation Year graphs: an entry is included if the injury date falls within the date range.
    • For other Background Question-related graphs, an entry is included if it is associated with an Incident Report and the incident date on the incident report falls within the date range.

  • DATA PARAMETERS EXPLANATION:
    • Injury and Graduation Year graphs: an entry is included if the injury date falls within the date range.
    • For other Background Question-related graphs, an entry is included if it is associated with an Incident Report and the incident date on the incident report falls within the date range.
INJURY COUNT BY ORGANIZATION
  • PURPOSE: For showing the number Injuries by Organization, with Injury Type listed; no PHI listed on report
  • DATA PARAMETERS EXPLANATION: An injury is included if the injury date falls within the date range
INJURY COUNT BY TEAM
  • PURPOSE: For showing the number Injuries by Team, with Injury Type listed; no PHI listed on report
  • DATA PARAMETERS EXPLANATION: An injury is included if the injury date falls within the date range
INJURY COUNT PER ORGANIZATION
  • PURPOSE: Able to pull data per affected area/injury type for a specific organization in graph format 
  • DATA PARAMETERS EXPLANATION: An Injury/Injuries are included by filtering which Organization, Affected Area, Secondary Filter, and Injury Type
INJURY TREND PER ORGANIZATON
  • PURPOSE: Shows any trends in specific injuries per organization and date range selected in a line graph format
  • DATA PARAMETER EXPLANATION: Injury trends are included by filtering which Organization, Affected Area, Secondary Filter, and Injury Type. Select which time interval will display on the chart (Daily, Weekly, Monthly, or Yearly)

    SURVEYS

CRITICAL SURVEYS
  • PURPOSE: For providers utilizing SAFE surveys to track how many critical responses have been received within the specified date range along with information like response time and follow up. 
  • DATA PARAMETERS EXPLANATION: Tracks from the date the survey was sent.
FAILED SURVEY SENDS
  • PURPOSE: For providers utilizing SAFE surveys to track how many surveys failed to be sent along with the name of the patient, the error code and the error message. 
  • DATA PARAMETERS EXPLANATION: Tracks from the date the survey was sent.

OTHER REPORTS

FORM DATA
  • PURPOSE: For providers who have Custom Forms; this report is generated by extracting the data of your choosing from your Custom Form(s)
  • DATA PARAMETERS EXPLANATION: An entry is returned if it was created within the date range
EMERGENCY CONTACTS BY ORGANIZATION
  • PURPOSE: For creating a list of Emergency Contact Info, and other important demographics by organization, including primary and secondary insurance information; No injury information is on this report.
EMERGENCY CONTACTS BY TEAM
  • PURPOSE: For creating a list of Emergency Contact Info, and other important demographics by team, including primary and secondary insurance information; No injury information is on this report.
TEAM INJURY REPORT
  • PURPOSE: For showing coaches which athletes on a team are injured and what their status is; only shows Athletes with a health status other than “Active”
  • DATA PARAMETERS EXPLANATION: the injury report for the current date is returned
MEDICAL DETAIL REPORT
  • PURPOSE: Provides the athletic trainer or healthcare provider with profile details of the athlete including important medical details. For example, if an athlete is allergic to anything - those details will populate in this report - as long as the athlete’s profile includes this info. To add this info, simply click on “Edit” while viewing an athlete profile and scroll to the bottom left to add information in the “Important Medical Info” field. 
  • This is a great report to travel with to quickly be able to view on the go which athlete has important medical detail. This can be printed as a paper copy to travel with or can be pulled up easily on your phone as well. 
  • DATA PARAMETERS EXPLANATION: This same as “emergency contact by team” but includes important medical details for athletes. While the “Emergency Contacts By Team” is accessible by Coaches, this particular report is only for Providers, as it contains personal health information that not every coach may not be privy to.
