Creating a Survey Action Plan

Surveys do not have to have an action plan. The provider may review results via the Survey Summary Report. If you wish to be notified of the survey completion, you will need to set an action plan.
There can be multiple action plans per survey

  1. Scroll to the bottom of the page for the survey you wish to create an action plan for. Select Add Action Plan
  2. Name your action plan
  3. Set your condition. The condition will determine what component on the form will be responsible for triggering the alert to the provider. There can be multiple conditions if you chose.
The below is an example where a Hidden Field was used on the form and titled Action Plan. In this instance the completion of the survey no matter the response will trigger an alert to the provider to review the results.
  1. Next set the follow up tasks for the provider. You can set as many Tasks as you like.
  2. Set the urgency level for the provider.
  3. Complete the information that goes along with your task selection.
  4. Select add an automated action to set the action for the provider and patient
  5. Set the parameters for your survey action plan
  6. Select Done
  7. You may set your survey schedule at this time or come back to it later.
  8. Select Update Survey
