Editing Form Data Reporting

If you’re generating reports from previously completed documents and notice that no information is being pulled, it’s often due to fields with identical titles. It's important to understand how to edit the form to make sure form data is pulling accurate and valuable data for you.

Form: The structure/build is designed to collect user information. This is essentially the template of the document type that is to be completed.

Document Type: The actual assignment of a form that can be completed by a user.

You will now see your form listing. 00:51: I'm going to edit the 20 24 2025 insurance information, 00:55: document by selecting, the menu icon to the right of the form. 01:00: Now, select edit. 01:03: On my form, I am requesting the name and date of birth of the user completing 01:08: the form, as well as the name and date of birth of the policyholder, these 01:12: may be the same and they may be different. However, I'm asking for them in the same format, 01:16: the title for the name is named. 01:18: Let's edit this for the user first. 01:22: You will see here. I'm going to type in athlete prior to name. 01:26: This is giving this label its own entity. 01:30: To see this action. I will now select safe. 01:34: Now as I had mentioned, I'm down in the policyholder information, you will see, 01:38: name is the label for this information as well. 01:40: I'm going to select edit for name here. 01:45: And similar to the previous step. I'm going to change this label to. 01:48: Now, how is the exact information I'm collecting? I'm going to change his label 01:53: to be policyholder name. 01:56: And again, select saved to save this edit. 02:00: As I have stated, I also have date of birth as duplicate Fields. 02:03: I'm going to edit those similar to how I did for the previous steps. 02:08: Here. I have change the label to be athlete date of birth, select 02:12: safe2save the faction. 02:15: And again here I'm going to change the date of birth for my policy, holder to 02:19: have a more appropriate title. 02:22: Here, I have change the title to hold policy holder, date of birth. 02:27: And select save to say this at it. 02:31: After verifying that your form contains, no duplicate labels clicksafe, 02:36: please know that any changes made within the form Builder will not retroactive 02:40: on those documents that have been previously completed by patient. 02:44: Rather, this will allow your report to pulling information of those 02:48: previously completed documents and it will change. 02:50: The visual documentation completion for those moving forward. 02:55: Now, let's navigate back to a report and see if our form data 02:59: data set is, now pulling an appropriately, select refresh. 03:04: Once you select it, refresh, the data should not be generated on your 03:08: report instead of it generating blank. 03:12: After our nightly, think you will be able to navigate back to your 03:16: form data report and view the actual changes within the 03:20: labels. Step-By-Step Tutorial:
  1. If you've created a report off of form data but note that the report is pulling blank this is your indication that the form may need to be adjusted
  2. Navigate to the document tab
  3. Select Forms
  4. Select the menu icon to the right of the form that needs edited and select edit
  5. Review your form for fields have have the same label. The form in the example has duplicated fields for Name and DOB. The first is for the patient information, and the second is for the insurance policyholder. These fields will need to be edited.
  6. Select the Edit icon for each field that needs a unique name
  7. Type in the new label
  8. Select save
  9. Repeat this action until all fields have their own unique label. Select Save to save your form.
    Note: As a reminder, any changes made to a form in the form builder will not change the format of previously completed documents. Any documents completed moving forward that have this form associated will have the new language on them.
  10. Navigate back to your report in Edit Mode, select Refresh
  11. Data will now be generated on your report
  12. After our nightly sync updates the reporting database, the labels will also be adjusted on the form data report.
