Sending a Survey - Web

Please Note: Terminology is different based on your setting, for example, a clinical setting uses Care Group as Organizations, Patient instead of Athlete, and Visits instead of Treatment, lastly Problems instead of Injuries, but the steps are still the same.

Step-By-Step Tutorial - Send Survey to Organization or Group

  1. Log in to your Healthy Roster account, and click on the Organizations tab on the left navigation bar

  1. Select the organization
  2. Click on the surveys tab

  1. Click on “send survey” next to the survey you would like to send

Step-By-Step Tutorial - Send Survey to Individual
  1. Locate the athlete's profile
  2. Click on the Surveys tab in the athlete's profile
  3. Click "Send Survey"
