Receiving and Complete A Survey as a Patient

As a patient, your provider or organization has sent you a survey for various purposes. You might be asked to complete a patient satisfaction questionnaire, a wellness screening, or even update annual documents. The goal of these surveys is to provide your provider with accurate, timely information to enhance your care experience and ensure their records stay up to date.

Step-By-Step Tutorial - SMS
  1. Click the link that is sent to you via text or email
  1. Complete the survey & select Submit at the bottom
  1. Based on the settings the provider has applied to the survey, you may receive a follow up text after completion

  1. Within your email select Complete Survey
  2. Complete the survey
  3. Select Submit
  4. A confirmation page will now show confirming completion
