Sending a Survey - App
Please note: You are only able to send a single survey through the app.
Please Note: Terminology is different based on your setting, for example, a clinical setting uses Care Group as Organizations, Patient instead of Athlete, and Visits instead of Treatment, lastly Problems instead of Injuries, but the steps are still the same.
Step-By-Step Tutorial
- Log in to your Healthy Roster app, and select the student you are sending the survey to
- In the student’s profile, tap on documents
- Tap the + sign in the top right hand corner of the screen
- Select “Send Survey”
- Tap on the box to select a survey, and tap on the survey you are wanting to send
- Double check the phone number is correct and tap “send survey”
