How to review survey responses

Reviewing Survey Results

Once a survey is completed by the patient, your action plans will be enacted. There are a few different ways you are able to review the survey results, we will detail each.

00:01: Hi, this is Nikki from healthy roster. 00:03: Once your patient has completed a survey you of course will want to 00:07: review the results. Most often surveys have action plans Associated 00:12: and those results will trigger the action plan, but that doesn't always have 00:16: to be the case this tutorial will cover how to review the survey results 00:20: action plan or not. 00:24: Let's first review. If your survey had an action plan that was set to trigger 00:28: a notification to the provider as that's most often the desired outcome 00:32: of the survey. So let the notification tab as you can see I 00:36: have an active notification. 00:44: As you see here, my patient has completed a survey and it's triggered my own 00:48: action plan notifications for action plans will be grouped 00:52: by urgency that meaning critical elevated or normal. 00:55: This is a way of helping the provider know which needs more of an immediate response. 01:00: Let's view this notification. 01:02: So let the menu icon 01:06: Now select open 01:09: Now that you've navigated to the response page we recommend scrolling to 01:13: the bottom. This will show you the actual completed survey so that 01:17: you can make sure you're reviewing all the survey responses. 01:27: Now you will want to complete the next steps for the survey. 01:30: These next steps will be the tasks that were set in the action plan. 01:33: Once you complete the instructions per step select the menu icon 01:37: to the right of the step. 01:41: And select Mark as complete. You will need to Mark each step as 01:45: completed in order to clear this notification from your notifications 01:49: tab. 01:51: Once you have completed all of the next steps. 01:54: Your notification for this survey response will now clear. 01:58: If you'd like to add additional notes, you can of course do so by selecting the add icon 02:03: and entering the note as you normally would. 02:06: As you can see here. I've navigated back to my notifications tab the 02:10: notification for the survey response that I have just cleared is now no longer visible. 02:19: Now let's review if you'd like to see these results in bulk action 02:24: plan or not select the document tab. 02:28: Now select surveys 02:31: Now you will want to select the menu icon to the right of the survey you wish to view. 02:38: Now select view data 02:41: Take a quick moment to familiarize yourself with this screen. 02:44: This is known as a survey summary report you're 02:49: able to change the filters to view the specific organization team, or 02:53: maybe even survey status that you wish to view. 02:57: I'm going to change my view. So it's only showing for the University 03:01: State College organization. And I would like to see only those 03:05: that are set for a requires follow-up. 03:11: I will now select the actions menu icon for grades Smith 03:15: in order to complete and review her survey results. 03:20: Select open 03:23: And once again similar to the previous steps, you'll be navigated to the survey 03:27: response page. I'm going to scroll to the bottom to review Grace 03:31: Smith's survey response. 03:41: Now once I've reviewed the full document, I will scroll back up to 03:45: the top of the survey results for Grace Smith. 03:48: Here you will see I have some steps that I need to complete select the 03:52: menu icon. 03:55: And select Mark as complete. 03:58: Once all of your next steps have been marked at complete the survey response 04:02: protocol will now be cleared. You will no longer see this notification as 04:07: an active survey that you need to respond to. 04:10: If you'd like to add additional notes select the plus button and add your note as 04:14: necessary. 04:16: I've now navigated back to my survey summary report page. 04:19: You will see here. I have the exact same filter still showing Grace 04:24: Smith is no longer there as we have cleared the requirement to review 04:28: her survey. 04:30: If you have questions or need more help, please visit the help tab on the left navigation 04:34: or reach us directly at help at healthy roster.com.

Step-By-Step Tutorial: Viewing Survey Notifications
  1. If your survey response protocols are set to trigger a notification, you will receive a notification once the survey is completed/the action plan is triggered. Select the notifications tab to view the notification.
  2. The notifications will then show. If the urgency level was set on your action plans, the results will be groups by the urgency.
  3. Select the menu icon to the right of the survey you wish to review, then select Open
  4. Scroll to the bottom of the page to review the survey's results
  5. Now complete the "Next Steps" for the survey; these steps will be the "tasks" that were set out in the action plan. Once you complete the instructions per each step, select the menu icon to the right of the step, and mark it as complete. If there is more than one step, you will need to mark each step as completed.
  6. Enter notes if desired.
Step-By-Step Tutorial: Viewing Survey Summary Report
  1. Select the document tab
  2. Select Surveys
  3. Select the menu icon to the right of the survey you wish to review, select View Data
  4. Ensure the filters are showing the correct Survey Data Report, adjust to your desired view.
  5. Review survey results
  6. Select the menu icon under Actions to review each survey response
  7. Select Open?????
  8. Scroll to the bottom to review the completed survey
  9. Complete any action steps
  10. Select the menu icon to the right of the step and select Mark As Complete
  11. Add notes if desired
  12. Return to your Organizations Survey Results page to continue to review survey results.
