00:00: Hi, this is Nikki from healthy roster. This tutorial is going to walk you through creating a form from scratch within our form Builder. This is an admin only feature. Another note to make is that this video will walk you through the fundamentals for building your form from scratch to detail your form. You will want to visit our additional linked training materials. 00:19: To begin select the document tab on the left navigation. 00:23: Now select forms 00:26: Select add in the upper right hand corner 00:29: To start a document from scratch select start from scratch. 00:33: Next you will need to name your form and give it a description. 00:37: Now you will begin to build your form by dragging and dropping the components onto the blank document. You will want to review three essential help docs form Builder editor tools form Builder Advanced editor components inform Builder tips and tricks. We will link them all below. 00:54: Once your form is complete or if you need to walk away select save. 01:00: If at any point, you need to edit or navigate back to the form itself select the three dots to the right. 01:06: This menu will allow you the ability to open edit or delete the form in question. 01:11: Once your form is complete, it will not show on patient profiles until you've assigned it as a document type to do. So, let's select the document types table. 01:20: To create a new document type now select add. 01:24: Now input the necessary information for your document type such as a title a description document management and assign the form. Let's review assigning the form next. 01:34: Under form options from the form dropdown select the form you created within the form build. 01:40: Continue on with the process of setting up your document type select save to assign to profiles. 01:46: The guide covered creating a form from scratch using Healthyroster. It included steps such as adding form details, customizing settings, managing components, and saving the form progress. By following these engaging instructions, users can effectively create tailored forms within the application.

Step-By-Step Tutorial:
  1. From the document tab select "Forms"
  2. Select Add
  3. Select Start from scratch!
  4. Provide your form with a Name and Description
  5. Begin to build your form by dragging and dropping the components onto the blank document. Review "Form Builder - Editor Tools" for a breakdown of the form builder tools.
  6. All drag and drop components are subject to further customization. "Form Builder - Advanced Editor Components" will break down the ways in which you can further customize your document.
  7. We encourage you to also reference "Form Builder - Tips & Tricks" as you are building your document to learn more about customizing your content.
  8. Once your document is complete or you need to walk away, select "Save"
  9. Your form will now be saved on your form listing. If you wish to edit the form, select the three dots to the left of the form name and select "edit"
  10. Once your document is complete, assign the form to a document type.
